sa\u011flanmas\u0131 i\u00e7in sa\u011fl\u0131k kurum ve kurulu\u015flar\u0131n\u0131n kurumsal performans\u0131n\u0131n \u00f6l\u00e7\u00fclmesine y\u00f6nelik olarak \u00f6l\u00e7\u00fclebilir ve kar\u015f\u0131la\u015ft\u0131r\u0131labilir performans, kalite ve di\u011fer kriterler ile hizmet sunum \u015fartlar\u0131n\u0131n belirlenmesi, halk\u0131n g\u00f6r\u00fc\u015flerinin bu s\u00fcrece yans\u0131t\u0131lmas\u0131 suretiyle kurumsal performans katsay\u0131s\u0131n\u0131n tespitine ve hizmet sunumu a\u00e7\u0131s\u0131ndan belgelendirilmesine ili\u015fkin usul ve esaslar\u0131 d\u00fczenlemektir.<\/div>\n<div><\/div>\n<div>Kapsam<\/div>\n<div>Madde \u00a02- \u00a0(1) \u00a0Bu \u00a0Y\u00f6nerge; \u00a0Sa\u011fl\u0131k \u00a0Bakanl\u0131\u011f\u0131na \u00a0ba\u011fl\u0131 \u00a0olmak \u00a0\u00fczere \u00a0birinci \u00a0basamak \u00a0sa\u011fl\u0131k<\/div>\n<div>kurulu\u015flar\u0131 ile ikinci ve \u00fc\u00e7\u00fcnc\u00fc basamak sa\u011fl\u0131k De\u011ferlendirici: Y\u00f6nerge kapsam\u0131nda kurum ve kurulu\u015flar\u0131 hizmet kalite standartlar\u0131 \u00e7er\u00e7evesinde de\u011ferlendirmek ve Y\u00f6nergede belirtilen di\u011fer i\u015f ve i\u015flemler i\u00e7in Ba\u015fkanl\u0131k\u00e7a g\u00f6revlendirilen personeli,<\/div>\n<div>l) Hizmet Kalite Belgesi: Bakanl\u0131k\u00e7a belirlenecek usul ve esaslara g\u00f6re Bakanl\u0131\u011fa ba\u011fl\u0131 sa\u011fl\u0131k kurum ve kurulu\u015flar\u0131 ve di\u011fer kamu, \u00fcniversite ve \u00f6zel sa\u011fl\u0131k kurumlar\u0131na verilecek belgeyi,<\/div>\n<div>m) HKS: Hizmet Kalite Standartlar\u0131n\u0131,<\/div>\n<div>ifade eder.<\/div>\n<div><\/div>\n<div>\u0130K\u0130NC\u0130 B\u00d6L\u00dcM<\/div>\n<div><\/div>\n<div>Il Performans \u00a0\u0130l \u00a0 \u00a0genelindeki \u00a0 \u00a0kurum \u00a0 \u00a0ve \u00a0 \u00a0kurulu\u015flarda \u00a0 \u00a0performans \u00a0 \u00a0y\u00f6netimi \u00a0 \u00a0ve \u00a0 \u00a0kalite \u00a0 \u00a0geli\u015ftirme<\/div>\n<div>uygulamalar\u0131n\u0131 \u00a0 \u00a0koordine \u00a0 \u00a0etmek, \u00a0 \u00a0izlemek \u00a0 \u00a0ve \u00a0 \u00a0de\u011ferlendirmek \u00a0 \u00a0amac\u0131yla \u00a0 \u00a0m\u00fcd\u00fcrl\u00fck \u00a0 \u00a0b\u00fcnyesinde koordinat\u00f6rl\u00fck kurulur.<\/div>\n<div>(2) \u00a0 \u00a0Koordinat\u00f6rl\u00fc\u011f\u00fcn g\u00f6revlerini y\u00fcr\u00fctebilmesi i\u00e7in m\u00fcd\u00fcrl\u00fck\u00e7e uygun fiziki mek\u00e2n, ara\u00e7-<\/div>\n<div>gere\u00e7 ve teknik donan\u0131m ve yeterli say\u0131da personel sa\u011flan\u0131r.<\/div>\n<div>(3) \u00a0 \u00a0Sa\u011fl\u0131k m\u00fcd\u00fcr\u00fcnce sa\u011fl\u0131k m\u00fcd\u00fcr yard\u0131mc\u0131lar\u0131ndan birisi Valili\u011fin teklifi Bakanl\u0131\u011f\u0131n onay\u0131<\/div>\n<div>ile il performans ve kalite koordinat\u00f6r\u00fc olarak g\u00f6revlendirilir.<\/div>\n<div>(4) \u00a0 \u00a0\u0130l performans ve kalite koordinat\u00f6r\u00fc ba\u015fkanl\u0131\u011f\u0131nda ildeki kalite y\u00f6netim direkt\u00f6rleri \u00fc\u00e7 ayda bir kez toplant\u0131 yapar. Bu toplant\u0131da \u00f6z de\u011ferlendirme sonu\u00e7lar\u0131, HKS\u05f3ye y\u00f6nelik iyi uygulamalar\u0131, HKS \u00e7er\u00e7evesinde yap\u0131lacak e\u011fitimleri ve direkt\u00f6rlerin g\u00f6r\u00fc\u015f ve \u00f6nerileri payla\u015f\u0131l\u0131r. Yap\u0131lan bu toplant\u0131 tutanaklar\u0131 elektronik ortamda Bakanl\u0131\u011fa bildirilir.<\/div>\n<div><\/div>\n<div>\u0130L Performans ve Kalite Koordinat\u00f6rl\u00fc\u011f\u00fcn\u00fcn G\u00f6revleri<\/div>\n<div>Madde 6- (1) \u0130l Performans ve Kalite Koordinat\u00f6rl\u00fc\u011f\u00fcn\u00fcn G\u00f6revleri \u015funlard\u0131r:<\/div>\n<div>a) \u00a0 \u00a0\u0130ldeki kurum ve kurulu\u015flar\u0131n performansa dayal\u0131 ek \u00f6deme uygulamalar\u0131n\u0131 takip etmek ve gerekti\u011finde istatistikleri Bakanl\u0131\u011fa ula\u015ft\u0131rmak,<\/div>\n<div>b) \u00a0 \u00a0\u0130lde memnuniyet anket \u00e7al\u0131\u015fmalar\u0131n\u0131 koordine etmek, \u00a0gerekti\u011finde anketleri yapmak veya yapt\u0131rmak,<\/div>\n<div>c) \u00a0 \u00a0\u0130ldeki kurum ve kurulu\u015flar\u0131 HKS \u00e7er\u00e7evesinde y\u0131lda 2 kez de\u011ferlendirmek,<\/div>\n<div>\u00e7) \u00a0 \u00a0Kurum \u00a0ve \u00a0kurulu\u015flar\u0131n \u00a0HKS \u00a0kapsam\u0131nda \u00a0yap\u0131lan \u00a0kalite \u00a0uygulamalar\u0131n\u0131n \u00a0izlenmesi, de\u011ferlendirilmesi ve koordinasyonu g\u00f6revini y\u00fcr\u00fctmek,<\/div>\n<div>d) \u00a0 \u00a0\u0130ldeki \u00a0 kurum \u00a0 ve \u00a0 kurulu\u015flarda \u00a0 HKS \u00a0 uygulamalar\u0131 \u00a0 ile \u00a0 hasta \u00a0 ve \u00a0 \u00e7al\u0131\u015fan \u00a0 g\u00fcvenli\u011fi uygulamalar\u0131n\u0131n desteklenmesi ve geli\u015ftirilmesi i\u00e7in \u00e7al\u0131\u015fmalar yapmak, bu konudaki uygulamalar\u0131n yayg\u0131nla\u015fmas\u0131n\u0131 sa\u011flamak,<\/div>\n<div>e) \u00a0 \u00a0G\u00f6revleri \u00a0d\u00e2hilinde \u00a0seminer, \u00a0konferans \u00a0gibi \u00a0etkinliklere \u00a0kat\u0131lmak, \u00a0g\u00fcncel \u00a0ve \u00a0mesleki yay\u0131nlar\u0131 izlemek ve ilgili yerlere duyurmak,<\/div>\n<div>f) \u00a0 \u00a0Koordinat\u00f6rl\u00fck b\u00fcnyesinde ar\u015fiv olu\u015fturmak,<\/div>\n<div>g) \u00a0 \u00a0Ba\u015fkanl\u0131k\u00e7a g\u00f6revlendirilecek De\u011ferlendiricilere gerekli deste\u011fi sa\u011flamak ve yap\u0131lacak \u00e7al\u0131\u015fmalara kat\u0131lmak,<\/div>\n<div>\u011f) \u00a0 \u00a0\u0130ldeki kurum ve kurulu\u015flar\u0131n de\u011ferlendirme verilerini Kurumsal Kalite Sistemine girmek.<\/div>\n<div><\/div>\n<div>Kalite Y\u00f6netim Birimi<\/div>\n<div>Madde 7-<\/div>\n<div>(1) \u00a0 \u00a0HKS uygulamalar\u0131n\u0131 y\u00fcr\u00fctmek \u00fczere kurumlarda \u201ckalite y\u00f6netim birimi\u201d kurulur.<\/div>\n<div>(2) \u00a0 \u00a0Kurumlarda \u00fcst y\u00f6netimin belirledi\u011fi bir \u00e7al\u0131\u015fan, Valili\u011fin teklifi Bakanl\u0131\u011f\u0131n onay\u0131 ile<\/div>\n<div>Kalite Y\u00f6netim Direkt\u00f6r\u00fc olarak g\u00f6revlendirilir.<\/div>\n<div>(3) \u00a0 \u00a0Kalite Y\u00f6netim Direkt\u00f6r\u00fc kalite y\u00f6netim biriminin sorumlusudur.<\/div>\n<div>(4) \u00a0 \u00a0Kalite \u00a0Y\u00f6netim \u00a0Direkt\u00f6r\u00fc; \u00a0Hasta \u00a0G\u00fcvenli\u011fi \u00a0Komitesi, \u00a0\u00c7al\u0131\u015fan \u00a0G\u00fcvenli\u011fi \u00a0Komitesi, Enfeksiyon Kontrol Komitesi, Tesis G\u00fcvenli\u011fi Komitesi ve E\u011fitim Komitesinin do\u011fal \u00fcyesidir.<\/div>\n<div><\/div>\n<div>Hizmet Kalite Katsay\u0131s\u0131<\/div>\n<div>Madde 8-<\/div>\n<div><\/div>\n<div>\u00dc\u00c7\u00dcNC\u00dc B\u00d6L\u00dcM Kurum Katsay\u0131lar\u0131<\/div>\n<div><\/div>\n<div>(1) Koordinat\u00f6rl\u00fck, her d\u00f6nem ilde bulunan kurumlar\u0131 ilgili d\u00f6nem sonuna kadar HKS \u00e7er\u00e7evesinde de\u011ferlendirmeye tabi tutarak (0,65) ile (1) aras\u0131nda de\u011fi\u015fen hizmet kalite katsay\u0131s\u0131n\u0131 tespit eder.<\/div>\n<div>Hizmet kalite katsay\u0131s\u0131 a\u015fa\u011f\u0131daki \u015fekilde hesaplan\u0131r:<\/div>\n<div>Hizmet Kalite Katsay\u0131s\u0131: De\u011ferlendirme sonucu elde edilen Toplam Puan\/(HKS Toplam Puan\u0131-<\/div>\n<div>De\u011ferlendirme D\u0131\u015f\u0131 Puan Toplam\u0131)<\/div>\n<div>(2) A\u011f\u0131z ve di\u015f sa\u011fl\u0131\u011f\u0131 merkezleri ADSM Hizmet Kalite Standartlar\u0131 \u00e7er\u00e7evesinde de\u011ferlendirilir.<\/div>\n<div>(3) \u00a0Bakanl\u0131k\u00e7a; HKS a\u00e7\u0131s\u0131ndan de\u011ferlendirilecek b\u00f6l\u00fcmlerin ilan edilmesi halinde sadece ilan edilen b\u00f6l\u00fcmlerde de\u011ferlendirme yap\u0131l\u0131r.<\/div>\n<div>Hizmet Kalite Katsay\u0131s\u0131 a\u015fa\u011f\u0131daki \u015fekilde hesaplan\u0131r.<\/div>\n<div>Hizmet Kalite Katsay\u0131s\u0131 = (Belirlenen B\u00f6l\u00fcm ve Standartlardan Elde edilen Puan \/ Belirlenen B\u00f6l\u00fcm ve Standartlar\u0131n Toplam Puan\u0131- Belirlenen B\u00f6l\u00fcm ve Standartlar\u0131n De\u011ferlendirme D\u0131\u015f\u0131 Toplam Puan\u0131)<\/div>\n<div>(4) Koordinat\u00f6rl\u00fck\u00e7e tespit edilen hizmet kalite katsay\u0131s\u0131 resmi bir yaz\u0131 ile bir sonraki d\u00f6nemin ilk ay\u0131n\u0131n \u00a0 son \u00a0 g\u00fcn\u00fcne \u00a0 kadar \u00a0 kurumlara \u00a0 tebli\u011f \u00a0 edilir. \u00a0 Koordinat\u00f6rl\u00fck\u00e7e \u00a0 kurumlarda \u00a0 de\u011ferlendirme yap\u0131lamad\u0131\u011f\u0131 durumlarda (koordinat\u00f6r gerek\u00e7esini tutanakla tespit eder ve il sa\u011fl\u0131k m\u00fcd\u00fcr\u00fcn\u00fcn onay\u0131na sunar) kurumun kurumsal performans katsay\u0131s\u0131 hesab\u0131nda hizmet kalite katsay\u0131s\u0131 kullan\u0131lmaz.<\/div>\n<div>(5) Hizmet kalite katsay\u0131s\u0131n\u0131n tespitine esas de\u011ferlendirmenin Ba\u015fkanl\u0131k\u00e7a yap\u0131lmas\u0131 veya yapt\u0131r\u0131lmas\u0131 esast\u0131r. Ba\u015fkanl\u0131k, kurumlarda bir sonraki d\u00f6nemin ilk ay\u0131n\u0131n 25 inci g\u00fcn\u00fcn\u00fcn sonuna kadar, hizmet kalite katsay\u0131s\u0131n\u0131n tespitine esas de\u011ferlendirme yapabilir.<\/div>\n<div>(6) Bir sonraki d\u00f6nemin ilk ay\u0131n\u0131n 25 inci g\u00fcn\u00fcn\u00fcn sonuna kadar Ba\u015fkanl\u0131k\u00e7a katsay\u0131 tespit edilmesi halinde Ba\u015fkanl\u0131k taraf\u0131ndan belirlenen katsay\u0131 ge\u00e7erlidir. Bu \u015fekilde belirlenen katsay\u0131 de\u011ferlendiriciler, koordinat\u00f6r ve ba\u015fhekim taraf\u0131ndan tutanak alt\u0131na al\u0131n\u0131r. Bu katsay\u0131, koordinat\u00f6rl\u00fck\u00e7e resmi bir yaz\u0131 ile bir sonraki d\u00f6nemin ilk ay\u0131n\u0131n son g\u00fcn\u00fcne kadar kurumlara tebli\u011f edilir.<\/div>\n<div>(7) HKS de\u011ferlendirmesi Ba\u015fkanl\u0131k\u00e7a belirlenecek olan \u00e7al\u0131\u015fma usul ve esaslar\u0131 ile rehberler \u00e7er\u00e7evesinde yap\u0131l\u0131r.<\/div>\n<div>(8) Hizmet Kalite Katsay\u0131s\u0131n\u0131n (0,65)\u05f3in alt\u0131nda olmas\u0131 durumunda Hizmet Kalite Katsay\u0131s\u0131 (0,65)<\/div>\n<div>olarak esas al\u0131nmak suretiyle kurumsal performans katsay\u0131s\u0131 hesaplan\u0131r.<\/div>\n<div><\/div>\n<div>Memnuniyet Katsay\u0131s\u0131<\/div>\n<div>Madde 9-<\/div>\n<div>(1) Kurumlar, her d\u00f6nem memnuniyet anketleri uygulama rehberi \u00e7er\u00e7evesinde standart anket<\/div>\n<div>setlerini kullanarak anketleri uygular. Bu \u015fekilde belirlenen katsay\u0131 de\u011ferlendiriciler, koordinat\u00f6r ve ba\u015fhekim taraf\u0131ndan tutanak alt\u0131na al\u0131n\u0131r.<\/div>\n<div>(4) Tespit edilen katsay\u0131 koordinat\u00f6rl\u00fck\u00e7e tutanak alt\u0131na al\u0131narak kurumsal performans katsay\u0131s\u0131 hesaplanmas\u0131nda esas al\u0131n\u0131r.<\/div>\n<div>(5) \u00a0 \u00a0112 \u00a0 \u00a0Hizmet \u00a0 \u00a0Kalite \u00a0 \u00a0Standartlar\u0131 \u00a0 \u00a0de\u011ferlendirilmesi \u00a0 \u00a0Ba\u015fkanl\u0131k\u00e7a \u00a0 \u00a0yay\u0131nlanacak \u00a0 \u00a0rehber \u00e7er\u00e7evesinde yap\u0131l\u0131r.<\/div>\n<div>(6) Ba\u015fkanl\u0131k, de\u011ferlendirme d\u00f6nemleri d\u0131\u015f\u0131nda da resen 112 hizmet kalite katsay\u0131s\u0131n\u0131n tespitine esas de\u011ferlendirme yapabilir. Ba\u015fkanl\u0131k\u00e7a, de\u011ferlendirme d\u00f6nemleri d\u0131\u015f\u0131nda resen de\u011ferlendirme yap\u0131lmas\u0131 halinde; de\u011ferlendirme neticesinde tespit edilen 112 hizmet kalite katsay\u0131s\u0131 esas al\u0131nmak suretiyle \u00a0ilgili \u00a0d\u00f6nemin \u00a0geriye \u00a0kalan \u00a0aylar\u0131 \u00a0i\u00e7in \u00a0kurumsal \u00a0performans \u00a0katsay\u0131s\u0131 \u00a0yeniden \u00a0hesaplan\u0131r. Bulunan kurumsal performans katsay\u0131s\u0131 ilgili d\u00f6nemin geriye kalan aylar\u0131nda Y\u00f6netmelik uyar\u0131nca da\u011f\u0131t\u0131labilecek ek \u00f6deme tutar\u0131n\u0131n belirlenmesinde kullan\u0131l\u0131r.<\/div>\n<div><\/div>\n<div>BE\u015e\u0130NC\u0130 B\u00d6L\u00dcM<\/div>\n<div>Kurumsal Performans Katsay\u0131s\u0131 ve Di\u011fer H\u00fck\u00fcmler<\/div>\n<div>Kurumsal Performans Katsay\u0131s\u0131<\/div>\n<div>Madde 12-<\/div>\n<div>(1) Kurumsal performans katsay\u0131s\u0131 kurum ve kurulu\u015flarda, en ge\u00e7 bir sonraki d\u00f6nemin ilk ay\u0131n\u0131n son g\u00fcn\u00fcne kadar kesinle\u015ftirilir.<\/div>\n<div>(2) Ayl\u0131k Kurumsal Performans Katsay\u0131s\u0131 kurumlar\u0131n Y\u00f6netmelik uyar\u0131nca da\u011f\u0131t\u0131labilecek ek<\/div>\n<div>\u00f6deme tutar\u0131n\u0131n belirlenmesinde kullan\u0131l\u0131r.<\/div>\n<div><\/div>\n<div>(3) Kurumsal performans katsay\u0131s\u0131 a\u015fa\u011f\u0131daki \u015fekilde hesaplan\u0131r:<\/div>\n<div>a) Kurumlarda;<\/div>\n<div>Kurumsal performans katsay\u0131s\u0131 = [(Hizmet Kalite Katsay\u0131s\u0131 x 0,65) + (Memnuniyet Katsay\u0131s\u0131 x<\/div>\n<div>0,20) + (Veri \u0130zleme Katsay\u0131s\u0131 x 0,15) ]<\/div>\n<div>b) Kurulu\u015flarda;<\/div>\n<div>Kurumsal performans katsay\u0131s\u0131 = 112 Hizmet Kalite Katsay\u0131s\u0131<\/div>\n<div>(4) Sa\u011fl\u0131k hizmeti sunumuna yeni ba\u015flayan kurumlar ile kurulu\u015flarda, i\u00e7inde bulunulan d\u00f6nem ve sonraki iki d\u00f6nem i\u00e7in kurumsal performans katsay\u0131s\u0131 (1) olarak kabul edilir.<\/div>\n<div>a) D\u00f6nem i\u00e7inde kurum taraf\u0131ndan verilen sa\u011fl\u0131k hizmetlerinin bir ba\u015fka hizmet binas\u0131nda veya yerle\u015fkede verilmeye ba\u015flanmas\u0131 durumunda ilgili kurumlarda bir sonraki d\u00f6nem i\u00e7in kurumsal performans katsay\u0131s\u0131 (1) olarak kabul edilir.<\/div>\n<div>b) Entegre il\u00e7e hastanesi iken stat\u00fcs\u00fc de\u011fi\u015fen kurumlar\u0131n i\u00e7inde bulunulan d\u00f6nem ve bir sonraki<\/div>\n<div>d\u00f6nem i\u00e7in kurumsal performans katsay\u0131s\u0131 (1) olarak kabul edilir.<\/div>\n<div>c) \u00a0 \u00a0Yang\u0131n, \u00a0sel, \u00a0deprem \u00a0gibi \u00a0do\u011fal \u00a0afetlere \u00a0maruz \u00a0kalan \u00a0kurumlarda \u00a0kurumsal \u00a0performans<\/div>\n<div>katsay\u0131s\u0131, i\u00e7inde bulunulan d\u00f6nem ve bir sonraki d\u00f6nem (1) olarak kabul edilir.<\/div>\n<div>d) \u201cSa\u011fl\u0131k \u00a0Bakanl\u0131\u011f\u0131na Ba\u011fl\u0131 \u00a0Sa\u011fl\u0131k \u00a0Tesisleri \u00a0Ve \u00dcniversitelere Ait \u00a0\u0130lgili \u00a0Birimlerin \u00a0Birlikte Kullan\u0131m\u0131 \u00a0Ve \u00a0\u0130\u015fbirli\u011fi \u00a0Usul \u00a0Ve \u00a0Esaslar\u0131 \u00a0Hakk\u0131nda \u00a0Y\u00f6netmelik\u201d \u00a0kapsam\u0131nda \u00a0birlikte \u00a0sa\u011fl\u0131k \u00a0hizmeti sunulan kurumlarda, birlikte kullan\u0131m\u0131n ba\u015flad\u0131\u011f\u0131 tarihten itibaren i\u00e7inde bulunulan d\u00f6nem ve sonraki iki d\u00f6nem i\u00e7in kurumsal performans katsay\u0131s\u0131 (1) kabul edilir.<\/div>\n<div>(5) Kurumlarda bu maddenin \u00fc\u00e7\u00fcnc\u00fc f\u0131kras\u0131 uyar\u0131nca her bir d\u00f6nem i\u00e7in hesaplanan \u201ckurumsal performans katsay\u0131s\u0131\u201d belirlendikten sonra hesaplanan katsay\u0131dan, her bir ay i\u00e7in Y\u00f6nergeye ekli Ek (6) \u201cFinansal De\u011ferlendirme G\u00f6stergesi\u201d do\u011frultusunda tespit edilen \u201cFinansal De\u011ferlendirme Katsay\u0131s\u0131\u201d \u00e7\u0131kar\u0131lmak suretiyle her ay i\u00e7in ayr\u0131 bir \u201cAyl\u0131k Kurumsal Performans Katsay\u0131s\u0131\u201d hesaplan\u0131r. Ayl\u0131k kurumsal performans katsay\u0131s\u0131, kurumlar\u0131n Y\u00f6netmelik uyar\u0131nca da\u011f\u0131t\u0131labilecek ek \u00f6deme tutar\u0131n\u0131n belirlenmesinde kullan\u0131l\u0131r. Tutana\u011f\u0131n bir \u00f6rne\u011fi il performans ve kalite koordinat\u00f6rl\u00fc\u011f\u00fcne g\u00f6nderilir.<\/div>\n<div>c) Kurulu\u015flarda bir sonraki d\u00f6nemin ilk ay\u0131n\u0131n son g\u00fcn\u00fcne kadar sa\u011fl\u0131k m\u00fcd\u00fcr\u00fc, il performans ve kalite koordinat\u00f6r\u00fc, iki \u015fube m\u00fcd\u00fcr\u00fc ve bir koordinat\u00f6rl\u00fck personeli taraf\u0131ndan bu Y\u00f6nerge esaslar\u0131na g\u00f6re hesaplanan kurumsal performans katsay\u0131s\u0131 tutanakla tespit edilir. Bu \u015fekilde kurumsal performans katsay\u0131s\u0131 kesinlik kazan\u0131r.<\/div>\n<div>\u00e7) M\u00fcd\u00fcrl\u00fckler taraf\u0131ndan, Kurumsal Performans Katsay\u0131s\u0131na esas veriler bir sonraki d\u00f6nemin ilk ay\u0131n\u0131n en ge\u00e7 son haftas\u0131na kadar Kurumsal Kalite Sistemine kaydedilir.<\/div>\n<div>d) \u00a0De\u011ferlendirmelerde, \u00a0gerek\u00e7eleri \u00a0belirtilmek \u00a0kayd\u0131yla \u00a0HKS\u05f3de \u00a0yer \u00a0alan \u00a0b\u00f6l\u00fcm \u00a0ve\/veya standartlar de\u011ferlendirme d\u0131\u015f\u0131 b\u0131rak\u0131labilir.<\/div>\n<div>(2) Finansal De\u011ferlendirme Katsay\u0131s\u0131n\u0131n hesaplanmas\u0131na esas te\u015fkil edecek bor\u00e7 verileri takip eden ay\u0131n ilk i\u015f g\u00fcn\u00fc Tek D\u00fczen Muhasebe Sisteminden al\u0131nacakt\u0131r. Toplam Tahakkuk verileri, ilgili saymanl\u0131k taraf\u0131ndan kurumun tahakkuk verileri Tek D\u00fczen Muhasebe sistemine girildikten sonra, Ek \u00d6deme D\u00f6ner Sermaye Komisyonu toplanmadan \u00f6nce Tek D\u00fczen Muhasebe sisteminden al\u0131nacakt\u0131r. Finansal De\u011ferlendirme Katsay\u0131s\u0131 Ek \u00d6deme D\u00f6ner Sermaye Komisyonu toplant\u0131s\u0131ndan \u00f6nce kesinle\u015ftirilir. Sa\u011fl\u0131k hizmeti sunumuna yeni ba\u015flayan kurumlarda, hizmet sunumunun ba\u015flad\u0131\u011f\u0131 tarihten itibaren 12 ay boyunca finansal de\u011ferlendirme katsay\u0131s\u0131 (0) olarak kabul edilir.<\/div>\n<div><\/div>\n<div>Di\u011fer H\u00fck\u00fcmler<\/div>\n<div>Madde 14-<\/div>\n<div>(1) \u00a0 \u00a0Ba\u015fkanl\u0131k\u00e7a belirlenecek usul ve esaslar uyar\u0131nca gerekli \u015fartlar\u0131 kar\u015f\u0131layanlara Bakanl\u0131k\u00e7a<\/div>\n<div>\u201cHizmet Kalite Belgesi\u201d verilir.
